• The Look of Love Free Prizes & Giveaways feature the following free items that participants may choose from: $10 Mielle Organics e-gift card, $10 Walmart School Supplies e-gift card, or a $5 e-gift card from the following merchants: Baskin Robbins, Burger King, or Taco Bell. When funding allows, and with pre-approval, we also provide a free food e-gift card, or other services for participants. The first 20 form entries will receive a free e-gift card via email at the email address they provided on the form.

• Instructions: Simply complete the form by providing your initials, email address, your state of residence for program tracking purposes, and your e-gift card selection. The first 20 form entries are automatic winners and will receive a free e-gift card via email at the email address you provided on the form. Parent or Guardian email is required for anyone under 18. You must be 18 or over to participate on your own. By completing the form and submitting your information, you are granting us permission to email you. You may unsubscribe at any time. • By selecting the acknowledgment above, by completing the form, and submitting you information for a free prize giveaways, the child's Legal Guardian, Physical Custodian, Case Manager or Adult Participant, or Client, agrees that he/she/they has read, fully understands, and agrees to the following terms: 1) The child’s Legal Guardian/Physical Custodian/Case Manager will provide an email address for the child to receive a free prize e-gift card via email. 2) The child’s Legal Guardian/Physical Custodian/Case Manager will provide full supervision of the child on the form. the website, and during the redemption of the e-gift card at the merchant or vendor. 3) The child’s Legal Guardian/Physical Custodian/Case Manager agrees to have child participants follow all program guidelines and understands that any violation on the child’s part may result in suspension and/or termination of The Look of Love's programs. 4) The child’s Legal Guardian/Physical Custodian/Case Manager hereby gives consent to participate in The Look of Love’s free prize& giveaway program. 5) The child’s Legal Guardian//Physical Custodian/Case Manager hereby agrees to release The Look of Love and Love Notes & News of all liability of injury, death, or other damages to the child, children, guardian, family, estate, heirs, or assigns that may result from his/her/their participation in the program, and hold harmless any staff, volunteer, participating merchant, vendor, company, facility, and/or other representative of The Look of Love and Love Notes & News, both collectively and individually, of any illness or injury, physical or emotional, other than where gross negligence has been determined. 6) At this time, per participant, The Look of Love may sponsor "up to $40" in free e-gift card prizes in total per participant. The first 20 form entries will receive a prize, then the game will be closed out. All prize notifications and e-gift cards will be sent to winners within 15 days from the date the game is closed out.

• Current and previous Board of Directors members are not eligible to receive a prize in this game.
